Primary Care/Family Practice Hours: 9am to 5pm Monday thru Friday (for scheduled routine and acute care)
Acute Care (please call ahead): Can be scheduled 24 hours prior or on the same day if appointment slots are available, Monday thru Friday. Please note that we do not take walk-in's.
Morning Blood Drawing service has been changed to between 8:00 and 8:45 am Tuesdays, Wednesdays & Thursdays
DOT physicals: 9am to 5pm weekdays scheduling is required & can be done within 24hr (appointments must be cancelled before appointment time to avoid a no-show charge.)
Urine drug screen hours: 9am to 6pm weekdays (specimen collected for DOT PE, work or voluntary screens only. We do not collect blood or urine for court ordered screens).
Services we provide include the following & more:
- Physical exams (Annual, school/sports, well child/baby, pre-op (EKG, blood work & chest X-ray available onsite), pre-employment, DOT physicals....etc.) Annual GYN exams are usually scheduled at 9am or 1pm.
- urine drug screening, pre-employment, D.O.T., sports & school physicals
- Preventive services (Papsmear, skin cancer screen)
- Treatment of minor injuries (lacerations/contusions, burns, sprains/strains, minor injuries....etc.)
- Diagnosis & management of simple fractures and sprains of extremities, casting & splinting
- Minor procedures (skin lesions/moles biopsy & excision, sebaceous cyst removal, ingrown nail removal, wart removal, IUD removal, injections for tennis elbow; shoulder/hip/knee bursitis; trigger finger, plantar fasciitis, wrist/shoulder tenosynovitis, trigger points....etc.)
- Evaluation/treatment of acute conditions (ear/nose/throat/lung/skin/bladder infections, poison ivy, allergic reactions, sexually transmitted diseases, stomach flu, reflux/ulcers....etc.)
- Management of chronic conditions (high blood pressure/cholesterol, diabetes, thyroid disorders, acne, eczema, allergy, asthma, depression, osteoporosis, arthritis, smoking cessation...etc.)
- Screening services (Quick Strep test, urine analysis, pulmonary function test, cholesterol/diabetes screening....etc.)
- Nursing Services: Depo-provera, allergy shots (administration only) & other therapeutic injections (Vitamine B12, etc.), PPD placement & reading, common vaccines administration, blood drawing (for LabCorp & Quest), urine drug screen...(Nursing services are office visits; if you have a co-pay for a doctor's visit, you are expected to pay a co-pay for nurse visit also.)
We accept: Medicare, all products of CareFirst BC/BS (including PPO/PPN, POS, Indemity, Blue Choice HMO), Cigna, UnitedHealthCare/MAMSI/ALLIANCE (including othe insurance payers using UHC/Alliance as agents, e.g. Coventry, Guardian, MetLife...), First Health/CCN (network includes MailHandler & a large numbers of other payers), CHAMPUS/TriCare for Life/TriCare Prime, GreatWest, PHCS, NCPPO and most PPO/PPN, POS, Indemity; worker's comp. & personal injuries. We offer reasonable fees for cash paying patients. Recent Changes: we have stopped aceepting new Aetna patients; services to existing Aetna patients will end after April 29th, 2013. We will stop accepting Optimal Choice and MDIPA (under the UHC umbrella as of January 1st, 2013).
All patients with insurance: please be advised that you must bring your current valid insurance card to your appointments every time for verification to allow us to submit claim to your insurance correctly. If your insurance company rejected our claim because you had failed to provide us updated insurance information (including name & address changes) you will be responsible for the full amount of the service you received. If you are unable to provide us with complete insurance information, you may still be seen but only as a cash paying patient.
Most commercial insurance subscribers have a co-pay for office visits. You are expected to pay your co-pay at the time of service. If you don't know your co-pay amount and we don't have your current correct insurance policy on file your co-pay will be $30 for that visit (you will later either be billed or credited for the difference)
Visits for cash paying patients starts at $80.00 (most initial visits will be over $90).
Personal checks will not be accepted from patients with history of bounced checks. A $25 fee will be charged for any bounced check.
You will be billed for $65.00 if you can't make to an appointment but didn't bother to cancel it. Please be considerated to other patients who may need your unused slot.
Our clinic is a family practice office; we are not a laboratory facility or vaccine clinic. We submit billing to your insurance company for blood work or vaccine services based on the rules your insurance companies set for a family practice provider. Our courtesy blood drawing service is considered an office visit; there may be applicable co-pay associated with the service.
Medication Refill Policy: Please allow 72 business hours after your phone request. We receive on the average 20 to 50 refill requests per day. Some refill requests require chart reviews to determine appropriateness of medication dosage, need for lab work, or clinic follow-ups. Refilling medications can be very time consuming. Dr. Jones works hard to answer the large volume of requests whenever physically possible. Please refrain from making repeated calls on the same day to check on the medication refill status. Our work flow is slowed down each time our very busy staff stop what they are doing to answer duplicate phone calls on the same request. In order to curtail these unnecessary interruptions, Dr. Jones has established new policy: the refill requests will be processed in the order when the latest request was received. This means if you left a message at 9 A.M. for medication refill, but called back at 3 P.M. to check on its status, if we haven't gotten to your request at the time of your second phone call, your request will be moved to behind every other request that came in before 3 P.M. In order to avoid the sense of emergency on your part, we encourage everyone to get into the habit of calling for refill(s) at least 1 week before the medication runs out.
Fortunately for us, vast majority of our patients are very considerate toward us as well as to their fellow patients. The above policy is intended only for the very select few with unrealistic expectations for others.
